Driver Consent Form - Sample

A sample form to provide employees and have signed to advise of audio and video recording within company vehicles:

Dear Valued Employee,

We have implemented a vehicle safety program with a company called HD Fleet. They are a technology partner with our company.  The primary function of this program is to identify No Fault accidents and address risky behaviors BEFORE incidents occur. This is done with a device installed on the windshield of our company vehicles that records audio and video while the vehicle is on and for 30 minutes after the ignition is turned off.  Audio and video will be recorded while using company vehicles.  

How does it work?

The device – an AI powered dash camera system – sends the safety team notifications when forces outside of normal driving are detected. Video clips will be automatically uploaded to an online platform.  These forces include speeding, harsh braking, extreme acceleration, impacts, and distracted driving (which itself includes smoking / vaping, smartphone use, eating / drinking while driving, no seatbelt, texting while driving, etc).  In addition to these notifications, the system will be used to identify risky behaviors such as distracted driving, unauthorized passengers, and driver fatigue. We will conduct “coaching sessions” based on the data to improve safe driving behaviors.

By operating one of our vehicles, you agree and consent to being recorded.

I acknowledge that I have read and understand this notice regarding the HD Fleet Event Recorders (dash cameras) installed in the company vehicles and I consent to being recorded.  Visual notices will be placed next to each device to inform all occupants / riders that there is a recording device onboard. This notice will look like this and will be placed on the interior windshield of each company vehicle.
